How do I redeem the $200 gift card for switching?

I switched over from Tmobile and brought 3 lines. How do I redeem the $200 Verizon gift card for each line brought over? It appears there are many that have had issues with redeeming the promo. Was this just bait to entice people to switch? It seems Verizon is making it very difficult to actually redeem and doesn't intend to honor the offer.
