What should I do if the ETF wasn't credited to my account?

My brother canceled his 3g service plan in October 2009 and was charged an ETF of $200 he later activated it on the condition that his ETF of $200 would be credited to his account. On January 6 2010 the service was interrupted for non payment and $135 was due I paid them $65 and I'm about to be billed again. I called them and they said there is no record the $200 was credited, I asked to speak to a manager the guy said there was no one available. what can I do to resolve this I really don't want to pay another ETF for canceling. Please advice me on what I can do in this situation.

Log into your "My Verizon" account and send them an email from there with a short, succinct explanation of what happened and ask them to correct it for you.
