I just got a replacement Droid X from Verizon and it has a blue triangle.

I think my phone is not activated and teh "onscreen" instructions never poped up lke the replacment guide said they would!

just Dial * 228 select option 1

Had the same issue just call to activate or do the factory reset and you will get the activate screen

The phone needs to be Re-Acvivated.
Dial *228 & choose option 1
Once completed Dial *228 once more and choose option 2 to update your signal
If that doesn't work you'll need to do a Factory Reset
Menu-->Settings-->Privacy-->Factory Data Rest--> Tap reset phone.
After its completed just follow the on screen instructions and you will be set
