What issues, if any, does the larger phone size cause with things such as where to carry the phone for ease of access to answer calls?

I haven't had any problem. I guess if you were a petitie female or someone with extrememly small hands you might. I think the size is blown out of proportion. Side by side the droid x is maybe 1/2" taller than the iphone. That's it. The way some people talk about it you'd think it was the size of an iPad.

The droid x seems to be an okay size. But I think that the Samsung Galaxy will even be more thin. Look at the one on T-mobile:

Don't get too stressed out about the size. The phone is a little bigger than a full-sized BlackBerry but its thinness makes it pretty easy to hold and even do some one handed holding/dialing. This is not a phone to have in a shirt pocket or the back pocket of your jeans - even though it would easily fit. I am using the beltloop holder that Verizon offers. Works very well - holds the phone safety and securely and allows very quick access.
To me the larger screen makes text entry and website reading so much easier that I am more than willing to put up with having to manage the larger phone size.

I have mine in my shirt pocket when I am walking around shopping, at work etc. In the car, it sits in my car dock. At home or when I am sitting at my desk at work, it is in the table.
I am planning on getting the holster for it in a couple of weeks
Before the Droid X came out, I was wondering if the phone was too big with that size screen. When Motorola finally posted the dimensions, I drew it out on a piece of paper, then cut it out.
