FIOS $150 Visa Prepaid Giftcard Missing

I became a verizon fios customer in March.  I was told by the sales representative that we would be receiving a $150 prepaid visa giftcard as a sign up bonus, however, it has not yet arrived and we have received no follow up communication.   I have a screenshot of my entire conversation with the representative, wherein he/she confirmed that we would be receiving it.
