HSI was escalated and called resolved

I had horrible HSI DSL, it was escalated. Over time Verizon seemed to fix the problem. They marked my case as resolved.
But now the large number of disconnects have started again and have continued for weeks.
Have once again restarted modem many times, restarted comp, checked to see all software was up-to-date, etc, etc.
Help Please?
