Verizon FIOS Dispute Resolution - Contract Proof Requested

Hello,
I recently contacted Verizon FIOS to cancel my service as I will be moving December 1st. I was informed I would be assessed an early termination fee due to the fact I am in contract. I requested written or verbal proof of my acceptance of the contract as I know I never agreed to another contract.
I have already filed a complaint with the New York State Attorney General and the New York Public Service Comission - Office of Consumer Affairs. I am posting here because I need the Verizon FIOS dispute address as well.
