Missing $100 Rewards Card

I signed up for Fios and was supposed to receive a $100 gift card.  The reward tile was on my homepage locked until I reached day 65.  On that day I received an email that said to click for my $100 reward card.  When I did this, the tile was now gone.  I have spoken to SEVERAL customer service agents and keep getting the run around.  It seems like everyone has this problem.  The last agent I spoke to said he would mail it out.  It's been over 30 days and still no card or response.
