Fios TV App and Fire TV

Hello,

We recently added streaming services to our Verizon package so I can stream from our office TV (no box top just internet enabled). 

The first two days of the service the TV worked fine, but now every time I log into the Fios TV app through Fire TV it forces me to deauthorize then reauthorize the Fire TV cube. However, the system will only allow you to authorize/deauthorize a device twice in one month. So, now I am unable to use the Fios Streaming service on this TV. 

Has anyone else come across this?

I've tried contacting Verizon to no avail. I've been told "you need to pay an additional $20 per month and a $25 equipment charge so that you don't have to authorize a device" and "you never received this service, you need to pay a one time activation fee of $69.99" even though we were given an install date and confirmation of install on 15 June.
