Context: Out of the box, cables (coax to router, HDMI to screen) connected, no observable function.
Behavior: powers on, displays two solid white LEDs for two seconds, no LED for six seconds, flashing white LEDs for six seconds, no LED for two seconds, green and red LEDs flash simultaneously for 60 seconds, unit powers off, cycle repeats.
Top button press yields no effect.

If the box is a regular VMS 4100, not the new WiFi based box you also need to have coax going from a splitter to the Fios box to get any programming. Regardless when the lights on the front of the box light up, you should at least get the built in screensaver. Are you sure your TV is on the right input? Did you try another HDMI cable? Can you just hook the box up to another TV via HDMI and see if you get a picture?

Tried with and without coax connected, two different screens, two different HDMI cables.

The lack of documentation is infuriating.

Sounds like you got a bad box. I'd call Verizon and arrange for a replacement. As far as documentation goes look here:
