After removing my Samsung soundbar from my Samsung tv, I am using the TV’s speakers, however I have to crank up the TV volume to almost 100 to hear anything reasonable. I replaced the hdmi cable and swapped out the inputs to the TV. The sound is great when just using apps like Netflix. It seems that the sound level is low when going through the STB on cable channels. Is there some setting in the user menu? I have pcm setting under the hdmi settings.

There is an audio setting in the STB that controls audio output.
Best thing to do is set it to the highest level and then set to fixed.

When switching to variable then trying to increase volume, the tv’ volume changes not the STB. I have the fios one mini boxes.

I unpaired the remote and set volume to max/30 and then set back to fixed but the issue still remains

Look here:
Main Menu > Settings > Audio & Video > Audio>Dynamic Range and set it to heavy.
Also look here:
Main Menu > Settings > Audio & Video > Audio>Audio Passthrough and set it to Passthrough.
Make sure the input settings for the input you have the Fios box connected to on your TV are correct and not set to a lower level.

Pass through is enabled and dynamic range is set to heavy. No change in the volume level.

The sound level of the set top box channels still seems to be lower than the app channels such as Netflix and others when using the internet .

Does the Samsung TV have any volume level settings for individual inputs like a receiver would have? I assume you have a Samsung TV.

There are 3 sound modes on the TV. Standard, Amplify and Optimized. I already have it set to Amplify.

Look in the input menu of the TV and see if there is a volume adjustment there for each input. What brand s\and model is the TV?

First make sure the Fios box is on Fixed audio mode and set to maximum.
Main Menu > Settings > Audio & Video > Audio>Volume Control
Looking at your manual, on page 75 is this on?
Auto Volume Try Now
Automatically adjusts the sound to a certain level when changing channels or switching to another external
input.
You could also try this:
Reset Sound Try Now
Is it possible that the Amplify mode only applies to the internal apps, and not the external devices? Try Standard mode and see if it helps.
Can you also check the level using another external source like a DVD player to see if the input the Fios box is on is OK?

It just seems like the sound going through the STB is the issue. I even got a new STB but the issue remains. I am going to revert back to getting a soundbar .

One more thing to try:
Turn off the box with the switch on top, then quickly press OK 2X on the remote. You should see a black and white list. Press the left arrow on the circle on the remote, you see a menu. Select USER, you should see some settings. One will be Additional settings. select it and you should see Audio Output, set it to PCM. Press Ok, turn off the box, and see if that helps.

Hmmm. Seems like you tried about everything I can think of. Funny nobody else has posted with this issue. Only thing left I can think of is to try setting the box to passthrough both in the service menu and the audio menu. Could also be the internal apps have a higher level than the external inputs. As far as I know unless the Fios box is defective, it should be outputting the standard up to 2V peak output. The only way to make it less is if the internal volume control is lowered, or ALL the program material is lower. Unfortunately the TV doesn't have an optical input, I'd try that too if you had one. I assume you tried the other HDMI input on the TV and had the same result. Is the issue there regardless of which channel you are watching?

My Samsung TV does have an optical input. Unfortunately, the Fios One mini STB does not have an optical output. I have 2 HDMI inputs on the TV and have tried them both.

I think that is an optical output according to the manual I read. Have you tried swapping the mini for a VMS Box?

The VMS Fios One box has an optical and analog out, the mini only has HDMI and analog audio out. Did you try swapping the mini for the VMS?
