On Demand Volume issue

I am a fairly new FIOS subscriber and have noticed that when watching On Demand videos in my bedroom - the volume fluctuates.  I have to constantly turn it up when it gets inaudible, then turn it back down when it is suddenly blasting at me.  This only appears to happen when I am watching something On Demand.  It is definitely an issue with the cable box or On Demand service and not my TV.  When I first noticed the problem I muted the TV volume and connected a small shelf system directly to the audio out on the cable box (Cisco, no DVR) and the problem still occurred - volume going up and down throughout the program.

Any thoughts?

So it seems that I'm having the same volume issues that were discussed here - While watching an On Demand video (such as The Voice) my volume will drop so low that I can't hear it, and then back up so loud that I'm concerned for the wellbeing of my speakers.  I constantly have to have the remote in my hand to adjust the volume, and its getting a little old.

I thought it was a problem with the box, so I had it exchanged for a new box.  New box now has the same issue, immediately after setting it up, and now after a week.  Suggestions?

My first thought would be that if you are using a device (TV, amp) that has automatic volume leveling (AVL), turn it off. This feature monitors the volume levels and quickly turns the volume lower if it perceives a loud event just took place. Then slowly returns the volume back to normal. However, if this is only occurring with On Demand, then AVL is probably not the cause.

I've been watching some Voice content On Demand recently and have not encountered this problem yet. I'm not sure how the On Demand servers work, but I guess the problem could be regional.
