cannot get HBO on one of our TV's

We have 2 TV's each hooked up to TIVO and each has a cable card. We have HBO as one of our paid channels. I can get HBO on one of the TVs but the other one says it is not available for my account.

In addition, we periodically get an error screen that we have to clear and it takes us right back to what we were watching.

So I am thinking this might be an issue with the cable card (which was replaced a year ago. I have had Verizon reset our cable cards remotely but that did not make a difference. Any Ideas?
