recently added an xbox one - on initial set up we did not connect the cable receive box (older version). Had a lot of trouble connecting a headset, but it did work. Then stopped. Technical support from xbox indicates that we should connect the cable box to the xbox with hdmi and then hdmi to the tv. Our cable box doesn't have an hdmi output. A) is this correct to get the headset to work - our volume setting is ghosted out on the xbox settings menu? or is there another solution?

That does not sound right to me. Some people might not even connected the xbox to a tv with a cable box. But if this is the case you can just upgrade to an hd box. Only a few bucks more a month.

Make sure the audio out of Xbox is set to bitstream. Hope that helps if not I’ll need more info starting with type of headset.
