Can't Redeem Amazon Echo

I signed up for Fios over a month ago and was sent the link to redeem my Echo. I clicked on the link which took me to Amazon and it said that the promotion code would be automatically applied, which it hasn't. Is anyone else having the same issue? Or know how to fix it?
