Cannot redeem Xbox gift card "the requested url was not found."

I have been unable to redeem my $50 Xbox gift card for around two months.  Each and every time, regardless of how I try to redeem it, I get an error stating: "The requested URL was not found in this server.  Go to MyVerizon Home."  I've tried clearing my cache, using multiple web browsers, using android and iOS phones plus the MyVerizon app.  I get emails saying the offer is available for redemption and have the tile in my Home offers tab.  I've spent hours with chat resulting in one instance of them telling me they would redeem it from their end and email it to me, then telling me that I had to wait two months after the installation versus the 30 days it states in the terms.  I've spoken with Gift Card Services, who can only offer me a Verizon gift card before transferring me to "Customer Service" which never seems to be available.

I see this issue and similar issues all over the forum and the internet and in nearly all instances it gets escalated to an agent with no follow up.  Has anyone found the problem and/or solution?  Verizon customer service has been unable to assist and seems more intent on working through their typical chat checklist then actually thinking about the issue.
