Did not receive $100 Fios welcome reward

I had Fios installed back in February. I became eligible for the $100 welcome reward in April. I followed the instructions I was given at the time and selected an electronic gift card. A little while later I received an email stating that my gift card would be mailed to me (makes sense!). It is now over 3 months later and I have received nothing. Can someone please assist as no FAQ or advice I have found has been useful to any extent.
