Link for reimbursement of cancellation fee not working

Hello,
I cancelled my Verizon landline/FIOS service today and the agent sent me an e-mail with a link to submit information about my move out of the service area in order to have the cancellation fee reimbursed. However, the provided link does not work. How can I get a working link and/or have the issue of the reimbursement of the cancellation fee resolved? Thanks in advance.
