Promotional gift card issue

 

Need assistance.  When I signed up for Fios gigabit internet it was advertised that I would receive a gift card for 200.00 after 60ish days of service. When logging in to my account couple times in past, showing the gift card was processing. Called rep couple times, told that it shows them shipped but have no information on processing as it never happened, rep mentioned of some issues.

 

Today when logged in, the tile is disappeared.  I would just like to claim and receive my egift card as advertised.
