Referral Information never added by Person taking order

Hello, 

I had referred a friend of mine for Verizon FIOS services.  FIOS was being newly installed in their residential area and a FIOS employee came to their house to personally take the order.  My friend called me at that time, so I could give him my information. 

It's been 2 months now since my friend got FIOS service. Recently when my friend called Verizon to inquire why I had not received my mastercard, she was told that there was no information on file regarding my referral. 

If  a customer service agent can send me a private message, I can provide the order #. associated with my referral.
