Trade-in devices lost and credit not applied to my number

I opened a family plan(4 lines) for verizon. I transfer to verizon from other company (T-mobile, etc). When i got the new devices,  I also got the mail box for trade-in. I sent my devices to the Fedex (all 4 devices in 1 box/package).  After few month, the credit applied to 1 of my account, which is abnormal. 4 devices should have 4 promotions. I call support, they found another devices, so they applied my another number promotion credit.  

How is it possible. 4 devices in one box, and support said they only found 2 devices. 

Its been about 5 month, i dont have the return trucking labels on my hand. I do have one device submission ID. 

I call the support 4 times, with no valuable feedback. Always told me I need to provide the trucking number. I only have one box (contains 4 devices) if you find another device. How is it possible to miss the rest 2 devices?

So frustrated to talk to the support.  (looks like verizon often lost trade in devices)
