Unreturned Equipment Charge Breakdown

Hi all,

When I disconnected my Verizon FiOS services earlier in the month, I found something online that broke down the specific equipment that I was supposed to return and the price for failing to return each item or returning each item damaged. After spending over an hour on the website, I can't seem to find that information on my account or anywhere else. Does anyone have any suggestions/helpful tips on where I can find this information?
