cannot activate rewards card

I received my Verizon rewards card 4 days ago and cannot activate it. I get a message 'we are unable to process your request' from the MyPrepaidCenter.com WEB site after I enter the card information. I get the same message when I call the 888 number. There is no way to speak to a person.
