missing reward/gift card

I received an email to redeem my $100 Prepaid Reward Card for my  order of Fios Home Internet: Gigabit Connection. I followed the instructions to redeem by clicking on the Redeem now button below. However, after doing so I do not see the reward tile to register my reward.  I been trying to figure this out since June.  Have had chat conversations about how to redeem but nothing.
