How do i register for prepaid reward card?

I placed my internert order in November and my first payment was taken from my bank account.  I still didn't receieve promised $50 welcome bonus prepaid card.  Anyone else having same problem? Thanks.  BTW the reward link is broken, it takes you to 404.
