Verizon Underground box Cover broken and needs replaced

The Verizon Underground Box Cover is broken and needs replaced on my property as it is a hazard at this point. The cover is 10 1/2 x 10 1/2. Please advise how to about getting a technician out to replace this as my landscaper almost fell in the hole last week when servicing my lawn. Thanks

You can report damaged Verizon property at 1-800-VERIZON.
