PUK code: SIM and Phone is Locked

I was messing around with security settings and thinking that I was setting up a SIM code I instead locked out my phone.  I followed instructions through My Verizon but when I click on "My PIN and Unblocking Key" it brings up a pop up but displays blank fields under Default PIN and PUK. 

Help!!!
