Transferring an important app

I'm so sorry for all the questions lately! I've already transferred my pictures, music and contacts to my new phone but there is one app that I want to transfer because it has all of my work out data saved to it. Is there anyway to transfer that app or info to my new phone without going to the store? Our store stays busy and I don't have the time to wait 2 hours. I hooked up both phones to my PC but I still can't find it. I know that is saved to my SD card on my old android phone. The app is called NOOM. It has all of my workouts and weight loss information on it. Thanks for all of your help!!!

Never mind, I figured it out. I had to download the app and do a back up to the website through the phone. Thanks!!
