Issue setting up Exchange ActiveSync Account?

I am unable to set up an Exchange ActiveSync Account for my work email.  I get an error that tells me to check my username and password - which are both correct.  I had the email set up and then deleted it off my phone when I went on vacation and couldn't re-set it up once I returned (this was on my HTC One.  I just upgraded to the HTC One M9 and am still having the issue.
