If you are on an unlimited plan then you may be getting intentionally slowed after 10Gb of usage.
Otherwise to report service problems you need to provide more info about the area impacted by the problem and then contact VZW directly. Info such as speed tests, current address and a few addresses in the area that can reproduce the same results are usually the first steps to getting a reaction from VZW.
We definitely want to get to the bottom of the speed problems that you are having. Let’s get this taken care of for you as quickly as possible. What Zip Code are you currently in? When did the problem with slow speeds start? Please describe what challenges you’re experiencing with the slower speeds.
