Android 5.0.2 on LG G2.
I used to get a prompt asking what file size, but my GF clicked the "don't ask me this again" button. Can't find it any settings for phone, email, or texting.

What do you mean, "What file size?" If you need to reset settings without a Factory Reset, Go to Settings-Backup and Reset-Reset Settings.

It's hard to remember now that I'm not getting the pop-up window prompt... I think I remember now that it was only when attaching an image to email, and the window offered several file size options (SM, MD, LG), listing the MB for each. Is there something short of a reset?

I performed the reset (non-factory reset) recommended and it did not bring
back the file size pop-up window for email attachments. It did however
delete all my preferences like wallpapers, etc.
Any other ideas?

I never had the stock email app ask me that before. I have never seen anything like what you are seeing on my G2 ever. Have you tried an app like Gmail or Outlook or K9, maybe it's just the stock app. I never had much luck with that app.

Yes, just running the stock email app which works fine. Can I just reload
that app?

You can clear the data and cache for it in the app manager, but you cannot reload it as in uninstall than reinstall it. It is a system app and cannot be removed nor downloaded.

1) Lame that it's not built into the app to adjust image size once the
pop-up is turned off.
