Phone Restarting

I updated to 48A last week and now suddenly it says my service is disabled and my phone keeps restarting.  Everything has been sluggish since the update, but now the phone doesn't work for more than a few seconds at a time before it restarts again.

1. What kind of phone do you have? (Manufacturer, Model)

2. Software Version? (Find this in the About Section of Settings)\

3. What 3rd party applications do you have installed? (Anything from Google Play, App Store, Windows Store, Amazon, etc)
