Still can't get my trade-in issue resolved after speaking to three Verizon reps

I originally traded in two iPhones on the same day to UPS. Verizon apparently only received one and credited me for just one.
So I chatted with one Verizon rep, then called in to speak to another and explained the situation. She seemed understanding enough, and processed the promo request. Thought that the issue was resolved right there.
A few months later.... nothing. My bill still doesn't show that the promo went through. So I chatted with another Verizon rep, who in a very passive way told me, "Since you don't have the submission ID, we can't do anything about it. Tough luck."
What? That makes no sense to me. I never received a submission ID for that trade in. I am beyond frustrated.
Is there anyone I can talk to at Verizon who has the authority to fix this simple issue so everyone can move on with their lives?

Since posting the above, I've had two more Verizon reps chat with me, and they are still giving me the run around.
At what point does someone at Verizon with any sense of customer service step in and actually help?
