Verizon not honoring Apple TV 4K Device Promotion

So on Nov 2023 I signed up with Verizon from a Costco kiosk and the offer says i'm eligible for 6 months free of charge Apple One bundle ($10/month value) AND an Apple TV 4K device (64GB version).
However, when i received my new phones (2 lines with Unlimited Plus plans, iPhone 15 pro max and iPhone 15 pro) and started the port-in process and I had to wait 2 days for port-in to be completed due to some issues (not from my side) and it seems like when they were fixing the port-in issues, they messed up my account and i lost whatever promotions were applied to my account when i signed up. So this started a very long customer support calls and follow ups to get these promotions back into my account, they did applied the Apple One bundle but as monthly credit BUT they never fixed the issues with Apple TV 4K device, I never got it till now, it’s been more than 3 months now and they just don't reply to me, they always says we will open a ticket to resolve the issue and expect to hear back from us with 5-7 business days, but got nothing! done this maybe 4 or 5 times so far.
Not sure how to handle this issue! i want what i was promised by Verizon! what is the next step(s) i can make beside calling customer service because they clear are not able help!! is there a place i can submit an official claim?
