Why do I have charges for 411 calls I didn't make?

I am being charged for 411 calls that I am not making. Seriously, is there any reason in today's day and age to make 411 calls? This has been going on for most of this year (started in Feb 2015). Many hours have been spent on the phone with Verizon reps and each month the charges have been reversed (thank you Verizon!). Yet this keeps on happening. The rep even got on a three-way call with Apple who said my phone is not making these calls. Yet this month the rep will not reverse the $70 charge, saying the calls are valid.This is incredibly frustrating as I have six lines with Verizon and this is the only one that continually causes problems. Help please!!! I don't want to have to take my service elsewhere, but I will if I have to because even though the $$ cost has been reversed, I cannot get these hours back on the phone.
