You can cancel the line by calling customer service at 1-800-922-0204 from a landline or *611 from your phone or you can visit a corporate store.
For the phone itself, backup your pictures and other personal information to your computer and factory reset the device. You can then sell it through Swappa or keep it in a drawer for a backup phone in case your current phone breaks.

Unfortunately I have heard that the trade in program offers less money than what the individual would get selling it themselves and there are too many horror stories of phones getting lost in processing or they were valued for less than the original appraisal.
