Why has my phone performed a factory reset on it's own (twice?)

The first time, I had left my phone on the coffee table while a friend was visiting with her toddler.  I assumed he had gotten hold of the phone while no one was looking and through chance had accidentally pressed the right combo to start a factory reset.

Today, I retrieved my phone from where it was plugged in over night and it prompted me to go through setup as if it had reset again.

This is not how your device should be acting! Let's take a further look at it:

Has anything changed on your device recently since this started? Any software updates or any new apps downloaded? Have you noticed any other errors pop up at all?
