error message sd card damaged or temporarily removed. How do I fix this. When card has not be touched?

Hoping someone can help.  SD has not been touched or damaged but still get messages that it is damaged or temporarily removed.  Then 4G disconnects and I have to power the phone off and then on to reconnect.  But still get same messages.  Anyone else having this problem?

Hmm that's odd, let's work together and get to the bottom of this mystery for you! Is this a new SD card? Have you tried to format your SD card since you started getting these error messages? http://vz.to/1hD5A1H
