You have to do it on the TV and on the Phone.
Enter the code shown on the phone into the tv.
Then the TV gives a confirmation code, enter that into the phone. Then you can exit out of the TV menus (or reenter to see the status change)
Then the Phone app should go to your tv listing.

Well I should say the Verizon Website didn't like Firefox.. After some discontent I used IE and I was able to register and confirm my iphone..
Freeking app is AWESOME!!!!!!
THANK YOU VERIZON!!!
p.s. so is the iphone going to VZW? No really, you can reply this to thread and I won't tell a soul.. 🙂

Anyone want to play the game of suspecting something now that Verizon has released an official app on the iPhone? Yeah I know this is Verizon and that is Verizon Wireless, but still. Are we close? Seems like it.

Ok i'm deleting my dvr access, and starting over and putting in the steps. WITH screenshots.
I'm running iPhone 3GS.
First of all, make sure you have an account at http://www.verizon.com/fiostvcentral
Make sure you have the latest verison of the APP.
Download it here on iTunes and sync your iPhone.
I'm connected via WIFI on my iPhone. I suggest you use that, and not 3G data (just for the setup).
Launch the App.

Select TV

Follow the directions Step 1-4.
On the STB, Press: MENU, SETTINGS, REMOTE DVR ACCES, MOBILE DEVICE ACCESS, press OK (if you have parental code setup, enter your PIN).

On Step 5 you will get a 10 digit code in RED on the phone. ENTER it into your STB. Press OK, it will say status: Awaiting Authentication.

On the Phone, Press next and it will remind you to enter the red numbers. Press OK.

Back on the STB look down and find the Confirmation Code (9 digits).

Enter it in the Phone and select Login.
You're done, the App on the phone should now download your DVR status and TV Guide.
On the STB you can press Exit to get out of the menus.

I finally was able to figure it out, what confused me was that the box to put in the activation code looked like it was for a phone number. I used the code that the app provided and put in the code the box gave me and that was it. Thanks again for your help!

Ok so I had this set up on my 3GS iphone and it worked great, but I upgraded to an Iphone 4 (with a different number) and now I can't get it to work. The TV gave me one code and the APP is giving me the previous code. I uninstalled and reinstalled it and it still won't work. Any ideas?

I use it on my Iphone 4 without issue. I suggest you go into the app on the STB and unregister your phone and reregister it.

Is anyone getting the new iPhone 4 app for Verizon DVR Manager 1.5.1? I am signing in as I do to my on-line Verizon account on the iPhone and the Warning box indicates that it is not recognizing my set-top box, that I need 1 to set this app up. And I have two and it does not recognize either of them. Verizon says it is the fault of Apple and Apple is pointing a finger at Verizon without any fixes in place. Got any ideas?

My wife has the FiOS DVR app v1.5.1 on her iPhone 3GS and it works great.
Does it let you login to your account? If so, what does it show when you go to Settings ? My Set-Top Boxes?

Updated version released today.
Version 1.1.1
Change Log:
* Provisioning Flow has been fixed for iPad and iPhone 0S 4.0 devices.
Also noticed that after I updated, I had to re-run the activation. Did it via the STB, took 30 seconds.
