Port Forwarding for CallerID/Fios App Access - Using my router with fios router

I sucessfully set up my router (direct eth connection from the ONT) with the Fios router behind it.  I put my router (modle # USG) on a 192.168.3.x subnet.  I have 2 STBs (192.168.1.100 and 192.168.1.101).  

My question is - can someone assist me on what ports need to be forwarded on my USG and what ports do I need forwarded on my Fios router?


Just a couple of notes, I did give my Fios router a static IP.  I did a reset on it durring the setup.  Now when I log into the Fios router I only see the following.

I am confused on what 192.168.1.102 is as I only have 2 STBs (.100 and .101).  Also, I cannot edit the above entries.

From what I have researched,  I see people referincing the follwing port forwards:

port 4567 TCP
63145 etc UDP
35000 etc TCP

Does anyone know if these are the only ports that need to be considered?  Also, any descriptions on how they need to be set up would be greatly appreciated.

My goal is to get back caller ID and be able to use the Fios mobile app to control the DVR (.100).
