I have the same situation. I contacted Verizon customer service. They basically told me that I should have received promo code on the first receipt and I should have applied for it in their rebate center with 30 days. Now that it is over 30 days they cannot do anything about it. I purchased their service in Sep 20. Please note, when I switched to their service, they never told me that I have to apply for the $150 egift card. I am on hold with their customer service for over 37 mins now and haven't had a chance to talk with human being there... I will update as I get any resolution. But, for sure, I will not let $150 go by just like that.
