Can't get account to switch phones

Hey guys, 

     I had to place a claim on my current phone and send it off for repair.  While it was gone, my SIM card was activated in an old phone (galaxy 5) that I had until my current phone was sent back to me.

My verizon account switched to the galaxy 5 but now refuses to update and show my current phone as the active phone.   This is a problem due to the line being insured. Does anyone have suggestions? I have tried multiple different variations of inserting the SIM with phones on/off.  A verizon employee reset the network.. and still nothing has worked.
