There are hundreds of post about activation failure with this new bundle. I had all the same problems and spent time trying to figure it out. It was finally solved. Here is what it was for me:
My verizon account I login with a username, but the account had an old email of mine in my profile. Once I activated the Disney bundle (in verizon), my guess it sends that email from your profile to Disney. Now, if you change that email in your profile cancel and reactive, it will not work. What worked for me was trying to login to Disney plus with that old email, click (forgot my password), it would send me an activation code, and once I reset the password, I have Disney, Hulu, and EPSN working.
Short answer - look at your verizon profile, whatever email is there, that is what Disney now has and use that to login. My guess is verizon sends it once only...and once set, that is it.
