Upgrade Despite Losing Phone?

Hello, I believe that I lost/ someone took my phone which was an Iphone5 that already had the two years with it complete. This means that it was eligible for an upgrade to a newer device as far as the plans go. I was wondering, since I lost the old phone, if it is required to have the old phone on you in order to upgrade? Meaning, can I just go to Verizon and ask to upgrade my device, even though I don't have the old one anymore? And since I lost it, I would not have the SIM card. Could I have the number of this new, upgraded phone be the same as my old,lost phone?

Call VZW at 8009220204 and report your phone lost/stolen.  Since you completed your 2 year contract you can go ahead and upgrade today.  Just remember that VZW charges an upgrade fee.
