Here is the scoop. My network extender is connected, all four LEDs are solid blue, yet calls are not going through the unit.
I have verified this by running the tcpdump program on my pfSense (FreeBSD based) router. I see keep alive traffic and other packets. But when I make a call there is no increase in network traffic. I still have poor phone quality.

What model extender do you have? Have you tried #48 to see if the Verizon network sees the call coming via the extender?

I'm was unaware of the #48. I tried it twice. A message to the effect "You are not under 4G coverage network extender coverage. Please move closer." I was under two meters from the extender.
Model SCS-2U01.
I need the extender only for voice calls. We live on the backside of hills from the towers and experience dropout and poor voice quality.

What make/model phone do you have? Do you have WiFi calling OFF? Has this ever worked? It could be the network extender has not been activated on the Verizon Network for your account. You could call 800-922-0204 or *611 and have someone verify/activate the extender MAC ID on your account.

You might look at this thread Network Extender where the phone had Advanced Calling/HD Voice turned on which is known to cause 3G extender connection problems.
