What good is this extender if the internet goes out which would be the only time I need it? When internet is up I can use WiFi calling and browse via my router. Does it need an internet connection after initial setup?

It absolutely needs the internet connection to work period. That is its function. It provides a cellular signal locally when one from the Verizon network is not available or weak and communicates to a network server for phone service over the internet connection. If your internet service goes out, then the extender's service goes out. If you have a WiFi capable device for calling then it does not need or use the extender's cellular signal. Without WiFi calling feature, a phone would need/use the extender's cellular signal for service via the internet.
