How do I fully close out a cancelled order?

I placed an order online for four new lines then cancelled -- my new devices did not arrive and were returned to shipper. Yet I continue to receive "your new line is activated" emails and my phone numbers are seen by Verizon as being "Verizon" numbers. I did not port any numbers in and in fact never received a device.
How do I fully ensure this order is closed out?
