Contract Buyout Claim Form Is Not Working

I received my final bill from Optimum and followed the link that was included in my 5G Home Internet Welcome email to upload so that I can be reimbursed for my early termination fee.
The page loads fine and I can see where I am supposed to fill out the information to submit, but most of the fields are grayed out and will not let me enter info.
The only fields that allow input are "termination fee amount" and "previous provider." I cannot enter my name, address or email address. I also cannot upload my documentation or submit. I have tried several times to speak with customer service but they seem to have no idea what I am talking about. I am specifically talking about this page:

Please help me your phone support doesn't have a clue what I'm talking about....
