Seems to be a trend here but no one seems to know how to send a return box for a modem that was never used. Does anyone have an address on where I can mail this thing that they claimed a box was sent 3 separate times but NO BOX WAS EVER SENT.

I don't know why CS can't just give us an address to ship equipment back to either as that would make things simpler for everyone. To answer your questions, you can ship equipment back in a box of your own so long as the items are sufficiently protected inside so they don't get banged around in shipment.
Go to Verizon's webpage and do a search for "return equipment". This will give you a page with loads of information on how to package your equipment as well as how to print the return label via My Verizon login to your account. But if you only had one service (I'm guessing it was internet or Fios in your case?) and that was disconnected, then you can't access your account anymore because login access gets revoked immediately upon cancelling all Verizon services. If that's the case, Verizon CS will have to pull up your account and hopefully they can send you a screenshot at least of a return label. They do hang out in these forums during the week, so hopefully one of them will PM you soon.

Hello, justsendthereturnbox. Help is here as you definitely deserve some answers. My apologies to hear that you have requested the return materials so many times to no avail. Just to confirm, the shipping materials we send come in an envelope as you receive the return label. You will need to supply your own box. Have you received anything from us that may look like junk/spam mail by chance?
