Someone hacking my account despite me having 2-factor authentication turned on

I have two-factor authentication turned on for my account, which means I have to approve any logins directly from my phone.  Despite that, someone was able to place an order on my account today for a new phone.  How is that even possible?

We recommend reaching out to the Fraud Prevention Team at 888-483-7200 so they can take a look at how this could have happened and make sure a solution is found.
